I have an older TE receiver which, -only- after 5-6 steady hours of use at a train show, refuses to operate in any direction other than reverse. I’ve never figured out why.
Turn the train around, that will fix it!
Actually one of our club members is working on an older TE transmitter that quit working. The switch in the controller’s forward switch needs replaced.
That sounds more like a receiver issue to me.
Also, I’ve been finding some bad “tactile” switches in things. You get the click, but not always contact. My doorbell is perhaps the worst.
I still have gremlins! Who wants them? FREE to a good home or bad I just want them gone. LOL
Yesterday running 2 trains while I worked on linking up a new runaround about every third trip a round one boxcar would derail over a switch. Why not everytime?? I’m convinced the GREMLINS were giving it a kick! I’m also convinced the old Aristo switch needs to have the FROG ground down a bit and I need to check the gauge on the wheels of the offending boxcar.

Have you checked the width of the wheels on the boxcar? One may be off slightly. It could also be the boxcar next to that boxcar in either side.
Some switches are really picky about wheel spacing and lubrication.
I was running all day yesterday on the club layout and had a separation of a train that was using Kadee couplers. It happened twice. I discovered a Kadee had “frozen” in the heat. Just gave it a little lubrication and it was happy.
